I support and deliver plans to improve primary care outcomes across a locality of practices providing equity of access to high quality services by patients. I am the named contact for a designated group of practices within a locality, providing support and guidance for the delivery of practice based plans that will enable the CCG to achieve its priorities and QIPP Plan. I also maintain effective relationships with practices, community providers, the CSU and the local authority is important to the role. In summary: • I Support the Senior Locality Leads to deliver local change programmes and primary care improvement at a practice level • I Act as a conduit for practices between CCG-wide, locality-based and individual practice application • I also act as the face of the CCG at meetings of partners and stakeholders (e.g. the local authority) and at public and patient engagement events.
