Previously, I worked as a care manager in the Community Care program at Mercy Home for Boys and Girls. As a care manager, I had the unique opportunity to work with former residents and their families. Through a systematic strength-based approach, I guided individuals and their families through the process of goal creation and obtainment. As well, build positive, structured relationships and provide opportunities for each member and their family to harness their potential. During my tenure at Mercy Home, I worked as a youth care worker in programs serving girls ages eleven through nineteen years old at Mercy Home for Boys and Girls. In this multi-faceted position, I supported youth in developing emotional, interpersonal, personal, and other essential life skills. As well, I assist and motivate youth academically. I counsel the youth by discussing their problems while encouraging them to remain positive through all times. As a full-time youth care worker, I provided support and resources to families to keep them involved in their children’s treatment. I was responsible for individual treatment plans, weekly advocate notes, weekly parent contacts, daily log entries, and other essential documentation. Overall, I enjoy partaking in enriching experiences with the youth and teaching them new things.   - Interview patients and family members to obtain information about their home, social environment, family relationships, health history, patient functioning, and treatment goals.- Explain the patient’s background, needs, and family dynamics to the team and participates in the planning of a comprehensive post-hospital care program for the patient.- Determine the most effective social work treatment modality and carries out therapy to facilitate the adjustments to disability and resolve psychosocial problems with the patient and/or family.- Collaborate with other healthcare professionals to develop, manage, and measure the effectiveness of the transition program.- Assist patients and families with community integration and referrals including DORS/DOA/Private Attendant Care Services, Home Health referrals, Continuum referrals, School reintegration, and Outpatient Services.- Identify eligibility and assists the family in applying for entitlements. Investigates alternative sources of funding for uncovered services in the community.- Participate in professional organizations and activities; assists in the orientation of other health care professionals in understanding the services and purpose of social work.- Assess patients for safety (suicidal/homicidal ideations/plans/gestures/means/intent).- Complete safety plans with patients when hospitalization is not warranted.
