I worked on a 1-2-1 basis with patients from the 6 Milton Keynes GP surgeries in Nexus Primary Care Network. I offered emotional and practical support for individuals struggling with social issues such as loneliness, housing concerns, and financial issues to empower them to improve their own health and wellbeing. This also involved supporting individuals struggling with their mental health, including anxiety, depression and suicidal thoughts. I liaised with other professionals to provide a holistic model of care specified to the individual’s needs and made referrals into other services where necessary.
