Peer Support Specialist
Current• Assists and provides services for individuals in need of support, including outreach,assessment, linkage and advocacy• Assists individuals in creating their own individual wellness and recovery plans.• Under the guidance of treatment team members, utilizes and teaches problem-solvingtechniques with individuals and groups.• Supports individuals’ vocational, psychosocial, daily living choices and skill building.• Assists individuals in building social networks in the community.• Transports individuals as necessary.• Completes agency documentation as required.• Participates in team meetings and provide peer perspective.• Collaborates and maintains relationships with community partners• Maintains appropriate and consistent interpersonal boundaries with individuals.• Maintains a caseload of clients• Conducts comprehensive assessments• Develops and implementing treatment plans• Provides substance use/co-occurring case management services
