CurrentCurrent Position: -Outreach to plan members to inform of missing HEDIS care gap measures -Assist in care navigation to improve health plan’s utilization metrics in preventative health services -Assess plan member’s Social Determinants of Health status (SDoH) and facilitate connections between plan members and community organizations to address SDoH barriers -Conduct home visits to improve utilization of member health plan benefits, improve member HEDIS care gap knowledge, and build rapport with community members-Train and mentor new employees on job expectations and daily job responsibilities -Collaborate with regional and department team members to problem solve and address workplace barriers
