Works under the supervision of the Executive Director to provide monitoring and evaluation of services and internal processes and strategies and plan quality improvement activities. In addition, the Quality Assurance Coordinator will work with the Executive Director to ensure compliance with state administrative rules and coordinate program audits. 1. Work with the Executive Director and Board of Directors in the development and coordination of a formal quality assurance/improvement plan;2. Conduct internal file reviews;3. Conduct customer satisfaction surveys and interviews; 4. Coordination of external ODDS audits;5. Participation in a Board led quality assurance/improvement activities;6. Upkeep of forms, including upkeep of electronic file folders;7. Development and upkeep of training material; 8. Review of all relevant OARs, policy transmittals, workers guides, FAQs, etc to ensure compliance with state/federal rules and guidelines;9. Attend training and create mini training10. Review PA performance measures;11. Participate and/or lead workgroups or projects as assigned12. Participate in internal and external meetings as appropriate13. All other duties as assigned 14. QA Coord. Position will be a 20 hour a week job with QA also carrying a caseload of 20 if full time staff.

Interviews customers, using Person Centered techniques (both formal and informal), who are eligible to determine their needs, provides crisis counseling when appropriate, and develops a plan of service for the individual and/or familyMakes referrals to outside agencies, describes agency and financial options to customers and/or families, and determines financial eligibility, guides customer and family through procedures.Counsels at least 40 customers individually, in family, or in other small groups regarding plans for meeting needs, and aids client to mobilize inner capacities and environmental resources to improve social functioning.Develop and monitor an individual support plan for each customer, visits customer and/or family at home when appropriate.Arranges for a variety of goods and services including but not limited to skills training, adult daily living supports, respite care, special medical or recreational services, etc.Helps customer to modify attitudes and patterns of behavior by increasing understanding of self, personal problems, and customer's part in creating them.Facilitates the development by community organizations of services needed by customers that don't currently exist or don't currently accommodate the specificneeds identifiedPrepares reports and provides statistical information as required.Reviews individual support plan and performs follow-up to determine quantity and quality of service provided customer and status of customer's case.Accesses and records customer and community resource information.Provides support consultation to at least 40 individuals, maintains regular contact with individuals and families and monitors progress of the individual support plan, visits the individual and/or family at home when appropriate.Maintains records of evaluations, individual support plans, contracts, job descriptions, billing documentation, referrals, service provision, case notes and follow-up reports for each individual.
