Provided patient-centered care in an interdisciplinary medical team floating within the inpatient medical units. Participated in family meetings, daily discharge rounds, and provided handoff to primary social worker. Conducted psychosocial assessments with patients and their support and implemented needed interventions including: discharge planning, grief and loss support, counseling, advocacy, and providing community resources and referrals.
