Provided assessment tools to evaluate consumers, document strengths and needs, and developed and implemented goals and objectives based on principles of normalization and the developmental model. Monitored, by personal contact, and by phone, the progress of all consumers assigned to supported employment. Maintained appropriate records to track consumer progress (hours worked, wages, progress notes and other programmatic date). Attended departmental staff meetings and special meetings as required.
