-Contact and develop leads on referrals for home health skilled and non-skilled services through daily interaction with physicians and discharge planners at hospitals and skilled nursing facilities.-Maintain sales territory and routinely exceeded monthly goals for referral generation while keeping a diverse payor mix for optimal reimbursement -Perform pre-assessment of patients prior to admission to evaluate all potential needs and develop rapport with facilities and patients-Develop brand strategies and perform in person marketing at resource fairs, senior centers, community events, etc. -Interact with potential clients and their families to ensure a positive and helpful experience.

-Assist in the supervision of Occupational, Speech and Physical Therapy staff under the direction of physician prescription for inpatient rehabilitation.-Communicate and assist the Benefits Coordinator regarding prior authorizations, authorizations for continued care, discharge planning and insurance appeals.-Order all DME for patients upon discharge, while assisting in the placement of patients to group homes or SNF depending on level of care required and insurance limitations. -Reviewed patient referrals prior to admit, to assess functional mobility and appropriateness for inpatient rehab admission-Responsible for therapy department billing and scheduling for daily treatments to maintain patient-first billing/treatment practices and hospital productivity.-Train new personnel and perform all necessary onboarding activities such as EHR setup, department rules/regulations and billing practices

-Performs therapeutic procedures such as functional ambulatory activities and therapeutic exercises to obtain goals set by the physical therapist.-Increases productivity of clinic by identifying opportunities to improve efficiency. -Manages client relationship with patient, communicating care plan and offering encouragement and support when necessary to maintain patient buy-in.-Collects and inputs data regarding patient progress with ROM, strength, balance and other quantitative measurements into daily note-Supports clinical staff with education on proper billing practices, timeliness of documentation and overall productivity to promote consistency among clinicians. -Progresses patient within scope of practice to facilitate highest outcomes for patient and physician.-Researches and implements most up to date, evidence based practices in orthopedic physical therapy and neurological rehabilitation.

-Collaborate with supervising Physical Therapist(s) to obtain highest patient outcomes possible and meet productivity goals set within region-Perform weekly updates with workman’s compensation patients and their case managers regarding work status and plan of care updates-Perform weekly community engagement in my role as our clinic’s outreach specialist, to promote access to physical therapy and our clinic in the region

-Provided at home personal training sessions with clients for general fitness development and promoting continuance of care with long term physical therapy goals. -Solicited new business through community outreach. -Maintained detailed exercise logs for documentation, liability purposes and progression of exercises.
