Complete comprehensive assessment to determine needs related to end of life including emotional support, coping skills, caregiver support/needs, financial resources and bereavement needs. Assist with advanced directives including DNR, living will and power of attorney for health care. Assess for safety in living situation and assist with nursing home placement if needed as well as respite stays.

Provided comprehensive psycho social case management to terminally ill patients and their caregivers as well as palliative patients. Provide case management and elder abuse investigatory functions within the agency to fulfill requirements of two state/federally funded programs. Case management includes assessing needs, creating care plans and coordinate services for seniors to ensure safety and independence in their homes. Elder Abuse Investigating includes implementing a case plan, coordinating services and working with the abuser and victim to improve their relationship while always keeping safety of the elder the number one priority

Assist the Social Workers with setting up appointments; travel to and from nursing homes, assisted living and long term care facilities to assess terminally ill patients’ psychosocial needs as well as other systems’ issues, i.e. familial, employment, insurance. Fulfilling timely visits, charting, and assessing needs per plans of care. Other duties include attending team meetings, assisting with death calls, bereavement work, admissions, and regular visits. Regular communication with team members to ensure quality care. Co-facilitate bereavement group, developing self-care group for employees, attend regular in-services to further assist in better service delivery/models. Developing knowledge of becoming vital team member through rapport with co-workers.
