Providing positive health dignity & prevention services (including STI screening/identification and management, medication adherence assessments, condom provision, family planning counseling services etc.) to all clients on ART to facilitate positive livingSupporting the scale up of various differentiated service delivery models in line with guidelines and client’s preferences with the provision of client treatment literacy and psychological support.Ensuring PLHIV on ART in my assigned facility are retained in care, track clients who missed appointments or are LTFU and status are updated on EMR.Supporting scale up of index testing services through tracking and testing of partners/ eligible children of index clients in the facility as well as community Facilitating client’s linkages for intra-facility, inter-facility and facility-community referrals as needed.Coordinating activities of the facility case management teams including documentation in the care cards, Registers, other relevant PMM tools as well as updating of clients records on the EMR and timely uploads on the NDR.Undertake in monthly and quarterly Data Quality Assessment (DQA) by verifying the availability, consistency and validity of the data, also ensuring good quality data using the standard operating tools.Working with clients, physician and peer health educators to develop an individual service coordination plan which includes realistic, measurable and mutually acceptable Goals that are based on the results of the initial assessment.Identifying and contact community-based organisations, primary care givers, housing and shelter services and other needed providers to establish referral agreements and coordinate collaborative efforts with clients.Educating Families on gender based violence and its impact on children, offer screening at every clinic visit and link GBV survivors to the appropriate CBOs.
