Conduct surveillance of reportable Infectious Diseases including outbreak investigations, conducting case patient interview and tracking with follow up suspected, confirmed cases. Collecting and analyzing data (descriptive and comprehensive analysis) to find the causes, risk factors of diseases or other health problems and interprets disease reports and other information. Active contributor to community health improvement through comprehensive roles in the Community Health Needs Assessment and the Community Health Improvement Plan for Lawrence County through collaboration with multidisciplinary teams to identify community health priorities, develop strategic plans, and implement targeted interventions.Analyze existing disease investigation, prevention, and control measures to recommend effective changes, enhance disease reporting, and update activities in line with CDC guidance.Actively involved in Epidemic response by monitoring status, conducting interviews, coordinating lab tests and vaccination campaigns, contact tracing, and ensuring accurate case reporting. Managed documentation, tracking, and reporting clusters and vaccine sites while adhering to updated CDC guidelines.Monitor and conduct weekly, monthly, and quarterly reports on infectious diseases at the county level and review state and national trends reports. Collaborate and communicate with health professionals to coordinate efforts.Coordinate outbreak investigation and response efforts, implementing measures to mitigate spread and impact.Attend quarterly regional partner meetings for the Healthy Choices, Healthy Communities Project and other meetings if needed like school nurse meeting, Board of Health Meeting. Communicate finding for update surveillance results.
