The Remote Cardiovascular Health program provides top of the line remote care to help diagnose and treat patients with hyperlipidemia and hypertension without directly seeing a physician. The goal is to improve cost effectiveness, quality of care and re-imagine the patient experience.- Directly spoke with and treated patients in regard to medication initiation, medication change, lab work / results and other beneficial therapies under the supervision of leading physicians in the field.

Assisted in launching Pediatric Asthma Intervention Pilot Program. Responsibilities included meeting with pediatric patients and their families and coordinated team development care plan to better manage the pediatric patient on a case by case basis. Contacts were across the care continuum and reported directly to the medical program director. Participated in activities and performed work related to the Greater Lowell Asthma Coalition and the Greater Lowell Health Alliance.

Assisted in a new program launched by Lowell General targeting diabeticpatient research and tracking. External care program put in place to lightenworkload of physicians through assistance of CNA’s and social workers.Responsibilities: Assigned patient list and communicated with themweekly to track and research their activity regarding medications, overallhealth and wellbeing. Compiled reports to summarize research. AssistedCNA or Social Worker with home visits to gather patient data for project.

Reporting directly to the Clinical Manager, reviewed cases for admission status, insurer status, discharge and location status, level of care documentation and management of patient database. As the lead, developed and launched new communications program to group of primary care physicians focused on notifications of medicare status for members who out-migrated on both an elective and emergent basis. Implemented closed loop process with PCPs on migration of admission and discharge information. Created documentation workflow and resources documents outlining programs and procedures.
