Duties: As a housing retention specialist my job duties are to provide direct comprehensive case management services to homeless or eminently homeless veterans, community development and advocacy. Screen potential client/veterans and/or veteran family members to determine preliminary eligibility for case management services per the SSVF grant, which provides financial assistance necessary to help keep veterans from becoming homeless or aid the already homeless with developing, coordinating and obtaining emergent, temporary and predominately permanent, safe housing. If eligible complete a comprehensive interview and psychosocial assessment with veterans and family members, in a therapeutic manner to obtain critical information necessary for service planning, daily living assistance, referral and advocacy, and crisis counseling/intervention. Based on veteran preferences/needs, which include complex, diverse, psychosocial problems, use community resources to coordinate services with other providers, by making appropriate referrals to community and other governmental agencies for services to promote ongoing and sustained stability. Per grant requirements compile a veteran charts, which include written documentation, such as progress notes, service plans, releases, screening records, psychosocial assessment records, resource and referral records, payment records as well as input housing information and services into the homeless management information system (HMIS) database. Additional case management responsibilities include assisting veterans in finding housing, ensuring reasonable rent is being charged, housing is safe per Housing and Urban Development (HUD) guidelines, resolve utility needs or issues, mediate problems with landlords and collaborate with other community agencies to coordinate services to ensure veterans and their families obtain and maintain housing.
