Oversaw the Health and Wellness Services program serving family caregivers, seniors and disabled adults through health promotion, case management and resource navigation. Managed the concept-to-launch of SCAN’s community services expansion in 2014. Helped increase individuals served from 2,700 to 18,000 in one year through active involvement in community events, capacity building, and collaborative partnerships. Led quality assurance work groups and business contingency planning projects. Built and managed high performing teams of resource specialists, social workers, nurses, health educators, event coordinators and data analysts. Ensured staff were compliant with budget/expenditures, adhered to regulatory and department requirements, and provided excellent customer service to people within and outside of the organization.

Provided clinical staff supervision and program administration of the California Department of Aging's MSSP (Multipurpose Senior Services Program) care management program, Medi-Cal’s home and community-based program serving frail older adults age 65 and over. Worked with management team and lead service providers to ensure best practices and quality of care was provided to all clients served. Ensured monthly census and operations met contract compliance and regulations. Developed and maintained policies and procedures, program budget, quality improvement practices and annual client satisfaction surveys.

Managed diverse caseload of Medi-Cal recipients living with multiple chronic conditions, limited functional abilities, and at-risk for hospitalization and/or nursing home stays. Conducted in-home psychosocial geriatric assessments, created client care plans and participated in weekly interdisciplinary clinical team meetings. Monitored client’s needs by maintaining face-to-face or telephone contact with client and/or client’s family members. Responsible for service planning and utilization review of services, caseload budget and ensuring care plan goals were met for each client.
