My position as a Home Coordinator at Catholic Charities I work adjacent to the Program Manager by taking her directive, communicating on a daily basis, and attending weekly supervisions to discuss updates. I am primarily responsible for scheduling and managing all appointments and extracurricular activities for multiple clients. I train new employees. I handle each clients monthly spending budget. While assisting clients during their health care visits, extensive notes, records of care provided, and follow up appointments are documented. During office hours, it is my duty to check in client’s medication, edit the Medication Administration Record (MAR) as needed, email updates about clients to their support team, fax or mail client information to appropriate recipients, and file paperwork. I devise tracking sheets and memos regarding changes in a client’s care plan using Microsoft Office templates. I assist the Program Manager by putting together and writing yearly reports on clients. To achieve success in this position, I keep a concise schedule of each client in my planner, set up calendars in each home as a way to communicate with other staff, make to-do lists weekly in order to prioritize what needs to be addressed, and use a binder to organize each client’s personal information. As a Home Coordinator, my position requires me to work both as a team and independently without direct supervision.
