Lead/Primary Contact of the BCBSM Physician Group Incentive Program (PGIP) -Manages all administrative duties for the WSUPG PGIP Physician Organization (PO) that includes WSUPG, University Pediatrics, and the Reynolds Radiology Group. The PGIP PO consist of over 680 Physicians (60 Primary Care Physicians & 620+ Specialist Care Physicians. -Revitalize the PCMH program at the 6 practice sites, consisting of 60 Primary Care Physicians and 100+ Residents & Fellows.-Creating educational material and conducting training sessions to clinic staff on the Patient Centered Medical Home (PCMH) program, reviewing the Interpretive Guidelines, the site visit process, and the designation process. -Coordinate the development of best practice documentation, process improvement work flows, and other performance improvement job aids -Direct communication to the 600+ Specialist Care Physicians (SCP) and 500+ Residents & Fellows on all specialty areas of PGIP. -Create and distribute SCP Value-Based Reimbursement reports; meeting with all specialists departments to present and discuss the program, selection process, and results. -Work with physician leads/champions on quality initiatives (HEDIS, Radiology, Women’s Care, Cardiac Care, etc) and creating action plan for improvement. Management of Meaningful Use (MU) Incentive Program -Oversaw all administrative duties for the 350+ eligible providers, including individual registrations in Medicare & Medicaid systems and complete all provider individual attestations. -Create, data validate, and distribute monthly reports for all 25 WSUPG departments, providing status on meeting MU measures. -Present MU information and updates at each department’s faculty meetings and scheduling needed training.-Completed and submitted hardship applications for Program Year 2014 & 2015, for applicable providers.

Value Partnerships, Field Team Representative •Support the development, administration and operations of the BCBSM Physician Group Incentive Program (PGIP).•Build and maintain relationships with senior leadership (i.e. medical directors, chief operating officers, executive directors, etc) and the physician community of Physician Organizations (POs) & Organized Systems of Care (OSCs) participating in PGIP. •Conduct Patient Centered Medical Home (PCMH) assessment and educational visits to primary care physician offices across the state of Michigan; while also offering consultative information to aid in the implementation of PCMH capabilities.•Assist and consult POs & OSCs on multiple areas including; -Adoption of quality improvement methods to facilitate chronic care management & population management of attributed patient lives. -Analyzing & evaluating PGIP data (BCBSM claim driven) that is received monthly, quarterly, and bi-annually. -Development of educational material for physician buy-in to increase the number of physicians in the PGIP program. -Offering further explanation and understanding regarding PGIP projects including PCMH & OSC Interpretive Guidelines, Specialist Incentives (BCBSM Uplifts), Supplemental Information projects, Medicare Advantage Gaps in Care, and PGIP Initiatives.•Maintain communication with assigned PGIP POs & OSCs on a regular basis (weekly, monthly, and quarterly (as applicable)) through emails, teleconferences, and in-person meetings. •Maintain up to date knowledge on HEDIS/MQIC Measures, State of Michigan Healthcare Environment (HIEs, POs, Hospital Community, etc), CMS, and the National Healthcare Reform as it relates and affects PGIP & BCBSM business & programs.

•Guide and direct ambulatory practices through the process of selection, adoption, implementation, and optimization of health information technology (HIT). •Direct ambulatory practices in implementing quality improvement interventions in response to Meaningful Use and other physician incentive programs (Physician Quality Reporting System (PQRS), Electronic Prescribing (eRx), Patient Centered Medical Home (PCMH), Hedis quality measures, etc).•Review and analyze ambulatory practices’ essential workflows in preparation of workflow process improvement using PDSA and LEAN methodology.•Develop operational plans to incorporate Care Management and evidence based practices, with emphasis on specific Meaningful Use and contract based objectives. •Track and analyze meaningful use metrics for assigned eligible professionals and identify process and performance gaps.

As a graduate intern with the Chicago Dept. of Public Health, I was able to contribute to the research of Emergency Preparedness & Response procedures, which would go into developing plans to ensure the safety of its large population of 2.5 million people, in the case of any disaster.
