Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes, through data sets such as HEDIS measures, member chart reviews, and collaboration with multidisciplinary teams

Assist healthcare providers in identifying missing or incomplete information in a patient’s medical record by performing retroactive chart reviews to ensure accurate diagnosis coding, hierarchical condition categories (HCC) documentation, and use of risk adjustment factor (RAF) scores to predict the cost of healthcare for a patient and a healthcare organization, support correct reimbursement, effective care delivery, quality improvement, targeted population health interventions, and medical research. Additional responsibilities included coordinating care with other providers, providing ongoing patient education, assisting patients in identifying barriers to care and providing support according to their needs, and conducting home visits to promote patient safety.
