Phase 1:• Educating patients on the disease process, heart healthy diet, exercise, and cardiac rehabilitation• Promoting independence through progressive ambulation, exercises, and developing skills such as climbing stairs, while monitoring their physiological response• Documenting ongoing plans for care and outcomes in EPICPhase 2: • Orienting new cardiac and pulmonary rehabilitation participants to program• Performing telemetry hookup, anthropometric measurements, medication review, clinical assessment, medical history review, monitoring of vitals, ECG review and interpretation, clinical documentation in EMR system, and exercise prescription and planning• Leading exercise stretching, free weights, and routine warm-up and cool-down session• Daily monitoring of patient blood pressure, pulse, and cardiac rhythm • Reporting and documenting patient progress, signs, and symptoms in clinical charts• Maintaining the knowledge and skills necessary to work with the geriatric and adult patient population and identifying the individual needs of each patient• Presenting educational lectures to patients focusing on the topic of exercise prescription for the cardiac patient• Developing and implementing care plans and ongoing review of outcomes• Ensuring a safe environment at all times for all participants by staying up to date with current hospital policies and program practices

•Interpret the results of periodic health and fitness assessments, including maximal graded exercise testing, and develop exercise programs for participants in a year-long university sponsored health program aimed at prevention and rehabilitation for lifestyle diseases such as heart disease and diabetes.•Supervise and assist clients in implementing exercise programs targeting their individual needs.•Educate clients on exercise safety and benefits of making healthy lifestyle changes.
