Consulted with members to identify their medical needs and recommended eligible benefits and programs to help create a personal medical care program for each one. Managed patient care by consulting with expectant members to advise on pregnancy care and assist with planning. Followed each patient’s case from beginning of pregnancy until post-partum.• Educated members on the benefits available to each one and assisted with enrollment into the case management program• Advised members as to what to expect during each stage of pregnancy, on the importance and expected frequency of attending OB Doctor appointments, and importance of following doctor’s orders• Worked with patients to create a proper diet and self-care plan during pregnancy to ensure proper nutrition for mother and child and safe delivery of the baby• Discussed possible complications and symptoms so that patient can bring issues to their doctor’s attention ASAP, reducing risk for mother and child
