Multifaceted role consisting of primarily identifying and implementing clinical development and performance improvement initiatives that enhance overall alignment and strategic clinical and operational partnerships between 11 Skilled Nursing Facilities and the major health systems (Northwell Health, Catholic Health System, Mount Sinai Health System, etc.) in the NY Metro area. Employs Improvement Science techniques such as Six Sigma, LEAN and PDSA to partner closely with Accountable Care Organizations (ACOs) and Post-Acute Collaboratives (PACs) around ensuring optimal resource utilization and clinical outcomes for patients receiving Rehab and Medical care in Skilled Nursing Facilities.

Lead a multidisciplinary team of 50 consisting of Managers, Supervisors and physical, occupational and speech therapists. Closely collaborate with hospital executive leadership as well as physician, nursing and case management leadership to optimize clinical outcomes, the patient experience, financial and operational performance and hospital throughput. Participant and team lead on multiple cross functional teams and performance improvement projects using Six Sigma, PDSA and SWOT Team methodologies. Support Rehab department Manager and Supervisors in ensuring readiness and compliance with regulatory agencies such as the Departmental of Health and Joint Commission on Accreditation of Healthcare Organizations. Manage the performance of the Rehab department leadership team by performing annual performance reviews and providing mentoring as a means of ensuring maximum staff engagement and to provide career development opportunities.

Manage a multidisciplinary team of 15-20 physical, occupational and speech therapists. Integrate on a daily basis with hospital, physician, nursing and case management staff and leadership to optimize clinical outcomes, the patient experience and hospital throughput. Participant and team lead on performance improvement projects using Six Sigma, PDSA and SWOT Team methodologies. Ensure readiness and compliance of team with regulatory agencies such as the Departmental of Health and Joint Commission on Accreditation of Healthcare Organizations. Perform annual performance reviews with specific and measurable performance goals on all senior therapists and supervisors. Act as a mentor to staff, senior therapists and supervisors in the service as a means of ensuring maximum staff engagement and to provide career development opportunities.
