Neurosurgery Sub-Internship
Overall Grade: HonorsOver July/August, I had the privilege to complete a 4-week subinternship at YNHH seeing patients across the Cushing, Nixdorf, Chief and Collins services with 1 week spent at Saint Raphael Campus. My daily routine commenced at 3am drafting patient notes, followed by pre-rounding the patients in preparation for rounds with the chief residents where I presented various patients. This involved detailed case discussions, neuroimaging interpretation and formulating management plans. I actively participated in over 40 cases in the OR, spanning the whole spectrum of neurosurgical subspecialties. I was involved through the entire continuum of care, from preoperative planning, during approach and wound closure, to postoperative care, constantly being tested on neuroanatomy, surgical instrument usage and pathophysiology. I am grateful to continue to develop my surgical skills from incision and electrocautery, to suturing and knot tying during case closure. I returned to NIR to further learn from the endovascular neurosurgeons I spent time with in May, further developing my IR skills including catheter handling, guidewire manipulation and angiogram interpretation.I had the privilege of conversing with and spending time amongst attending neurosurgeons who are masters in their fields, attending many of their clinics to see them in practice. The surgical techniques, patient care philosophies, and academic pursuits of each attending left an indelible impression on me. I attended neurosurgery conferences on a weekly basis including M&M’s, vascular, spine, tumor conferences as well as interdisciplinary conferences involving neuroradiology and pathology, also having the opportunity to give my own presentation on extended reality within cranial neurosurgery.This immersive experience has been a transformative journey of learning, development, and clinical exposure which I am excited to carry forward into the next steps of my journey into neurosurgery!

Throughout my preclinical years, I regularly conducted full body cadaver dissections, dry and wet laboratory work multiple times a week in fields encompassing molecular biology, cell biology, genetics, organic chemistry, inorganic chemistry, biochemistry, physics, histology, practical skills and observe various laboratories involved in many different fields of research.I intercalated in my 3rd year to complete my Neuroscience BSc where I studied modules covering Neural Computation, Neuropharmacology, Autonomic and Central Control of Cardiorespiratory Function, Neurobiology of Brain Injury and Investigative Methods in Neuroscience and wrote my dissertation on optogenetic techniques for vision restoration. I gained experience within the world of neuroscience and the different disciplines within.Beginning clinical medicine, I conducted my 4th year at Royal Free Hospital in 2021/22 with core clerkships in Neurology (4 weeks), Renal (3 weeks), ENT (1 week), Preparation for Practice (1 week), Hematology (2 weeks), Infectious Diseases (2 weeks), Acute Medicine & Endocrinology (6 weeks), Cardiology (3 weeks), Respiratory (3 weeks), General Surgery & Gastroenterology (6 weeks), Vascular Surgery (1 week), Orthopedics & Rheumatology (6 weeks) and Anesthesia & Perioperative Care (6 weeks).In my 5th year, I was based at University College London Hospital, with 9 weeks at Basildon University Hospital, rotating in Pediatrics (6 weeks – 1 week at Great Ormond Street Hospital), General Practice (6 weeks), Breast & Urology (3 weeks), Sexual Health & Dermatology (3 weeks), Obstetrics & Gynecology (6 weeks), Oncology & Palliative Care (1 week), Ophthalmology (2 weeks), Geriatrics (4 weeks) and Psychiatry (6 weeks).I spent my final year at Watford General Hospital rotating through core clerkships in Accident & Emergency (4 weeks), LTC (Geriatrics & Palliative Care) & Radiology (4 weeks), Medicine (4 weeks), Vascular Surgery (4 weeks) and General Surgery (4 weeks).
Neurosurgery Clinical Elective
I conducted a 4-week clinical elective with the Surgical Neurology branch of NINDS in NIH Clinical Center. I joined daily morning rounds with the junior and chief residents on the floor and ICU, preoperative consenting, assessing post-operative recovery, reviewing management plans and coordinating further investigations and imaging. I conducted independent consultations taking histories and performing examinations at weekly clinics. I was privileged to spend time with the exceptional NIH resident and attending neurosurgeons. I scrubbed into nearly every case participating in super-specialized cases pertaining to patients with rare genetic disorders, pathological manifestations and comorbid afflictions of multiple neurosurgical diseases. I was intensively immersed in every case, developing my experience and surgical skills from prepping and draping to registering neuronavigation, incision and exposure to knot tying and suturing, and wound closure to postoperative care. I was constantly educated on the intricacies of neuroanatomy, pathophysiology, surgical instruments and different neurosurgical approaches.I attended weekly lab meetings and visited laboratories, discussing and learning from the cutting-edge research projects taking place at NINDS, also giving my own presentation. I attended NIH Clinical Center Grand Rounds, NIH Clinical Neuroscience Grand Rounds, journal club, case conferences, seminars, practical workshops, talks and tutorials which covered a vast range of topics in biomedical research, bioinformatics, translational neuroscience, biotechnology, genomics, neuronal regeneration and many more. I also spent a few days at MedStar Georgetown University Hospital, Washington DC, where I joined the residents and attendings on rounds, consults and within the OR! I took time to traverse famous centers like the Natcher Museum and The Nobel Prize Laureate Exhibit Hall and The Johns Hopkins Suburban Hospital and Walter Reed National Military Medical Center!

I led the creation and delivery/teaching of clinical tutorials and mock OSCE’s focusing on clinical medicine to clinical year medical students.
Pals Tutor
As a peer assisted learning support (PALS) tutor to junior years, I gave regular tutorials on a variety of core preclinical conditions, using acquired feedback to improve my tutorials throughout the year.
Preclinical Education Committee
I led the team collaboratively responsible for delivering medical tutorials to preclinical students as well as organising mock OSCE station circuits for the consolidation of practical skills.

My tenure as President of UCL Malayali Society in 2021/22 was one of the most challenging and fulfilling roles I have embarked in at my time in university. After founding this society in the year prior, in this year with the help of my amazing committee, we built a cultural community from the ground up to spread and enhance Malayali culture throughout UCL. We held fortnightly events ranging from games nights, social gatherings, cultural balls, religious festivals to boat cruises with attendance of 300+ members as we became a prominent cultural society collaborating with many other societies around the country.

As founder and with the rest of my founding committee for this ethnic society for my culture of origin, I was involved in creating a cultural community at UCL where we organised fortnightly events dedicated to celebrating and spreading the food, dance, music, cinema, festivals and religion of the region. We created a sustainable foundation within an era of social distancing and lockdown, which future committees can build upon.
Neuroradiology Clinical Elective
During April and May, I spent 4 weeks at YNHH in Neuroradiology. I dedicated many hours in MR, CT, and NP reading rooms, learning to interpret various neuroimaging modalities. I independently used VISAGE, learning subtle signs in acute and chronic neuroradiology pathologies. I gained experience with CT, MRI, fMRI, US, myelography, CT/MR angiography, PET, and DAT/SPECT scans.I observed procedures like CT/fluoroscopy-guided lumbar punctures, head/neck/spine biopsies, CTA/MRA, myelograms, discograms, blood patches, nerve blocks, epidural steroid injections, and facet joint injections.In interventional radiology, I assisted with cases such as visceral organ biopsies, CVC insertions, gastrostomy and nephrostomy tube insertions, tumor ablation/chemoembolization/radioembolization (Y90), IVC filter placement/removal, balloon angioplasty of extremity arteries, endovascular bypass, varicose vein ablation, and mechanical thrombectomy of a pulmonary embolism. I also participated in endovascular neurosurgery cases, including cerebral angiograms, aneurysm coiling/embolization, intracranial artery dissection stenting & balloon angioplasty, and stroke thrombectomy. Throughout this week, I refined my IR skills, including suturing, catheter handling, guidewire manipulation, contrast injection techniques, radiation safety/dosimetry, and intraoperative CTA/MRA/US interpretation.Academically, I attended daily conferences including neuroradiology fellow’s conferences, radiology resident conferences, weekly interesting case conferences, and interdisciplinary conferences involving neurology, pathology, pediatrics, and neurosurgery (including tumor board, spine and vascular conferences), IR grand rounds on Wednesdays, and Radiology Grand Rounds on Thursdays. Highlights were presentations by Dr. Curtis Langlotz from Stanford on Neural Networks in Radiology, Dr. Andrei Holodny from MSK Cancer Center, and Dr. Veronica Chiang from Yale Neurosurgery on Gamma Knife and LiTT.
Neurosurgery Sub-Internship
Overall Grade: HonorsFrom March to April 2024, I embarked on a comprehensive 5-week subinternship in the Johns Hopkins Department of Neurosurgery at The Johns Hopkins Hospital, engaging in rotations across the Brem, Walker, Cushing, Dandy, and Pediatric services, with an additional week at Bayview Medical Center. Each day began early with rounds alongside junior and chief residents, followed by intensive patient care, management discussions, and involvement in diagnostic and procedural activities, significantly enhancing my skills in neuroanatomy, patient management, and interdisciplinary collaboration. I also undertook 4 24-hour on-call shifts and a full week of night shifts, providing me with extensive emergency care experience.I participated in 39 OR cases encompassing every neurosurgical subspecialty. I was intensively immersed in many different subspecialties, approaches and technologies from incision and exposure to suturing and knot tying for wound closure followed by postoperative care. It was a real-time, dynamic education in neuroanatomy, pathophysiology, surgical instrument usage and neurosurgical techniques. The academic components of my subinternship were equally enriching, with active participation in Grand Rounds, Cushing Rounds, a Subinternship Boot Camp, teaching conferences, and specialty board meetings, developing my understanding of neurosurgical practices. Communication, empathy, and teamwork were foundational aspects of my experience, along with conducting independent patient consultations during clinics. An enlightening day spent at The Hunterian Neurosurgical Laboratory with a postdoctorate research fellow allowed me to observe and engage in cutting-edge research on brain tumors, linking clinical expertise with scientific innovation. This subinternship was not only an educational journey but also a profound personal and professional development opportunity, reinforcing my passion and preparing me for a future in neurosurgery.
Neurosurgery Clinical Elective
A 4 week clinical elective in Neurosurgery at the National Hospital for Neurology and Neurosurgery (NHNN), Queen Square, London over October/November 2023. I had the opportunity to observe 50+ neurosurgical operations over 4 weeks, scrubbing in to 15+ cases and assisting with suctioning, retraction, hemostasis and wound closure, in subspecialties including neuro-oncology, spine, neurovascular, skull base, hydrocephalus, epilepsy, peripheral nerve and functional neurosurgery. I closely observed and examined a wide range of neurosurgical approaches including both conventional surgical techniques and minimally invasive techniques such as neuroendoscopy, with novel applications of medical technology for the investigation, diagnosis and management of a vast range of conditions, such as Stealth Station Electromagnetic Navigation, 4K-3D videomicroscopy, MR-guided focused US, robotic stereoelectroencephalography, computer/AI assisted planning and gamma knife radiosurgery to name a few. I observed and shadowed neurosurgeons across ward rounds, Gowers Grand Rounds, QS Brain Bank Clinicopathological Conferences, MDT meetings, clinics, preoperative and postoperative care interacting with patients performing history taking, examinations and bedside procedures. I also spent time in interventional neuroradiology, neurology and anesthesia gaining a holistic view of the profession. I was also an organizer and photographer at QS Simulation Centre for 3 RCS England accredited neurosurgical courses, which offered me deep insights and development in surgical skills and knowledge within various subspecialties and neurosurgery as a whole!

As Vice President of UCL Surgical Society this year, I have together with the President and the rest of the committee been involved in supporting all the different branches of the society in organization, leadership and communication in order to ensure everything runs effectively to uphold standards of previous years, is marketed well and receives good attendance and feedback.A few of the activities I have been involved in as VP is personally leading a Peer2Peer mentoring scheme as Mentor Coordinator between junior and senior years delivering support, education and advice, organizing a Surgical Skills Course weekend in partnership with The Surgeon Theory and organizing along with the rest of the iNUGSC committee, the largest led UK student conference of the year; iNUGSC 2023.
