As the Director of LGBTQIA+ relations, I am responsible for improving access and quality of care for our lesbian, gay, bisexual, transgender, and queer patients. Internally, I work with medical and operational leadership to increase the cohesion of care for our patients of this community. That includes systems improvement like improving data capture in our EMR, improving access to preventative medicine, and defining care for this community beyond that of just HIV. Externally, I work with community partners who offer services that we do not, e.g. social supports for youth and adults. Likewise, I work with health plans to reduce barriers for our patients who wish to have gender affirmation surgeries. By building relationship with other organizations and increasing capacity and quality of care internally, I strive to decrease disparities of the LGBTQIA+ community.As the Director of AmeriCorps Programming, I manage a team of dedicated, purpose driven young adults who volunteer a year of their lives to helping the marginalized communities we serve. Under my direction, members volunteer their time to expand beyond that which a health center can typically provide. I work with health center staff and the interests of the members to meet the need of the moment. That includes efforts like increasing cervical cancer screenings, assisting patients while getting the COVID vaccine, and increasing sexual health awareness for youth.

I manage the development of DSRIP projects within primary care, behavioral health, health home and other programs in order to reduce potentially avoidable hospital admissions over the DSRIP waiver period. This involves coordination of internal and external stakeholders in order to achieve practice transformation that will ultimately lead to improved patient clinical outcomes and satisfaction as well as a reduction in utilization of hospitals' inpatient units and emergency departments, resulting in a lower cost to the healthcare system. Main areas of focus include implementing health coaching into primary care, peer-led interventions for patients with HIV, rolling out the Collaborative Care (IMPACT Model) program in primary care, and coordinating directly with hospital and other partners around reducing barriers in the continuum of care, namely around substance use disorder and post-acute follow up.
