Serving clients by offering support and guidance to get clients connected to resources. Care Coordinators have a dual role that interacts directly with clients seeking services and with the partners that provide resources and support to the community. Care Coordinators correspond with service partners to ensure a seamless experience to get clients connected to services. Care Coordinators also conduct follow up contacts with clients to ensure clients were able to get assistance as well as identify any other needs clients may have.Care Coordinators are Unite Us Platform experts, are embedded in their community and can provide guidance to partners to support the connectivity with community resources (for example, senior centers, homeless shelters, food pantries). Client needs assessments are conducted to help identify which resource providers can best assist clients based on clients’ needs, eligibility, and geographic location. Resources that clients get assistance with include medical expenses, food assistance, and permanent housing.

Serving probationers to ensure that Court Orders are fulfilled and thatprobationer get all services necessary in the community and in any residential settings;motivating probationers to make any necessary changes to their lifestyle and motivatingthem to cooperate and be engaged in any necessary treatment; identifying risks andneeds for each probationer, and responding accordingly; have open communication withservice providers to identify treatment goals, barriers to reaching goals, and strategies toaddress said barriers; ensure that compliance is met regarding contacts withprobationers and service providers according to DOC minimum standards; holdingprobationers accountable for antisocial/criminal behavior and rewarding them for goodbehavior, whether the reward be tangible or just acknowledging good behavior andgiving verbal praise; collaborating with service providers and other organizations tobetter serve probationers; giving detailed documentation of every contact with theprobationer and/or service provider.Reason for leaving: I sought out other opportunities to assist people in different ways. My position at Unite Us allowed me to have those opportunities.

I held many different positions at the Department of Juvenile Justice including CHINS Probation Officer, Commitment Prevention Specialist, Diversion Specialist, and Intake Officer. Duties included working with particularly high risk youth who are eligible to be committed tothe Department of Juvenile Justice; serving youth and their families to ensure that CourtOrders are fulfilled and that both the youth and their families get all services necessary inthe community and in any residential, secure, less secure, and/or group home settings;motivating youth and their families to make any necessary changes to their lifestyle andmotivating them to cooperate and be engaged in any necessary treatment; identifyingrisks and needs for each youth, and responding accordingly; have open communicationwith service providers to identify treatment goals, barriers to reaching goals, andstrategies to address said barriers; ensure that compliance is met regarding contacts withyouth, parents, school officials, and service providers according to DJJ minimumstandards; holding clients accountable for anti-social behavior and rewarding them forgood behavior, whether the reward be tangible or just acknowledging good behavior andgiving verbal praise; collaborating with service providers and other organizations tobetter serve clients and their families.During this time, I was the leader of the Social Fund, which organized events for the officeincluding spring picnics and events during Probation and Parole Week. I was also theleader of the School Team which worked within Renaissance Academy in Virginia Beach.As School Team leader, I helped build working relationships between the VBCSU andRenaissance Academy staff and students.
