Care Coordinator
CurrentServing clients by offering support and guidance to get clients connected to resources. Care Coordinators have a dual role that interacts directly with clients seeking services and with the partners that provide resources and support to the community. Care Coordinators correspond with service partners to ensure a seamless experience to get clients connected to services. Care Coordinators also conduct follow up contacts with clients to ensure clients were able to get assistance as well as identify any other needs clients may have.Care Coordinators are Unite Us Platform experts, are embedded in their community and can provide guidance to partners to support the connectivity with community resources (for example, senior centers, homeless shelters, food pantries). Client needs assessments are conducted to help identify which resource providers can best assist clients based on clients’ needs, eligibility, and geographic location. Resources that clients get assistance with include medical expenses, food assistance, and permanent housing.
