In this role, I was responsible for a caseload of 32 individuals who participate in both HCS group and host home programs. I supervised 15 full-time house staff, and was required to call on PRN staff when needed to meet staffing needs. I worked daily with a multi-disciplinary team of service coordinators, nurses, community programs, day hab staff, and client families to ensure that each client is provided with the services he or she needs.I encouraged clients to maintain their autonomy, but was available to assist in obtaining employment, ensuring they are housed, medical needs are met, purchasing supplies such as hygiene and home products, and scheduling appointments. I completed IPC and revisions on an annual basis to ensure services are up to date, provide clients with units for services such as: adaptive aids, dental, behavioral support, dietary, nursing, physical/occupational therapy, supervised living, and others. I communicated daily with clients and their families, and was skilled in communicating at a level they can understand.

I directly support over 35 individuals with IDD and their families as an HCS Host Home Program Coordinator. I collaborated regularly with a diverse team, including their service coordinators, nursing staff, day hab staff, and other professionals to adeptly meet the needs. I assisted in completing annual IPCs by identifying behavioral needs and training goals, as well as other necessary services. I carefully managed my schedule in order to visit client homes on a quarterly basis to complete reviews and ensure homes meet state requirements. In 2018, Vita-Living transferred from a paper record-keeping system of SDL and behavior logs to utilizing TaskMaster Pro. I crafted a systematic training plan and system to train all providers on this new process and within 2 months all families were able to successfully use TaskMaster Pro to record their SDLs.

As Site Manager, I assisted in managing and training the caregiver team in conjunction with the Site Lead. I maintained enrollment, attendance, and financial records for all students. I built relationships with the students and parents to ensure a quality experience for all.
