Provides assistance in linking programs and services with the school for the purpose of improving school attendance, student performance, and positive youth development. Monitors education, health, and human services received by assigned students and their families, to ensure that they are delivered in a personal, accountable and coordinated manner. Coordinates the scheduling of activities and sites for programs. Attends and facilitates after school, evening, and weekend programs. Maintains records and documentation that demonstrates the level of service and achievement of outcomes identified through applicable extended services project. Coordinates and facilitates programs, services, special classes and extracurricular activities for students and their families. Attends meetings called by project directors and the school principal

• Assist in coordinating the implementation of the Crossover Youth Practice Model in Colorado, and Texas• Create and maintain a database of all CYPM site team members (past and current)• Track the consultant and team usage of the CYPM conference call lines and webinar services• Produce a monthly CYPM newsletter for issuance to all CYPM sites• Work with CYPM Research Director and staff to continuously improve ability to view data• Ensure completion and timely submission of the monthly team reports from consultants• Maintain routine contact with the CYPM program manager• Maintain the CYPM wiki website and the CYPM portion of the CJJR website and Data Entry using (MS Word, Excel, Access, etc

 Provide weekly, monthly and yearly reports about youth programs and opportunities. Identify and connect with community organizations, youth centers and other interested parties to recruit eligible youth. Assist with budget tracking and grant reporting Evaluate the needs of the youth in the community. Connect community to local resources, programs and training services. Collaborate with police department and juvenile court councilors Ensure that all programs and activities are implemented according to relevant legislation, policies and procedures. Recruit and manage volunteers Research and developed programs for youth based on community needs. Ensure that youth and youth organizations were aware of available activities. Identify youths’ needs and goals to develop a comprehensive intervention plan. Provide assistance to families in distress, ranging from accessing basic services to helping resolve family conflicts. Liaison between program clients and service providers/schools to facilitate client access to services. Assist clients who are seeking employment, from helping these youth develop résumés, to identifying their skills and qualifications, and helping them apply for work with workforce services programs. Conduct gang awareness presentations in schools. Assist in researching and identifying program funding sources. Case Management
