PROCAREFUL project aims to develop an innovative model for hybrid home care services to support independent living of older and vulnerable people in need of care, taking advantage of digitalization.
The project wants to overcome the challenge of the existing urban and rural gap in home care accessibility and timely delivery by upscaling the existing health and social care services and by moving them from a reactive towards proactive approach.
To achieve this, partners develop a strategy for digital transformation of home care remote support. The partners work on a solution that combines face-to-face care with remote and digital support based on machine learning and artificial intelligence. The deployment of the model is tested in five regions.
