Worked with community partners and volunteers to manage programs that have been developed in the areas of housing first, outreach, and employment; provided leadership and coordinate different partners to implement the programs; maintained project schedules and resources; monitored and reported status on projects; managed grants through communications with grantees, analyzing progress, and providing support to help grantees achieve their goals; maintained communications with all consultants and contractors; developed and negotiates the terms of contracts.• Strategically led a collaborative partnership in reestablishing the Hospital to Home program with goals to minimize the overuse of the emergency room by the homeless population with an estimated annual savings of $150,000. Partnerships include but not limited to Grady Health System, Georgia Regional Hospital, and Piedmont Hospital.• Developed a system to transition recipients of the Georgia Housing Voucher into safe, affordable housing through a partnership with the Department of Behavioral Health and Developmental Disabilities • Managed $860,000 of grants funded by the Atlanta Foundation and the David, Helen, and Marian Woodward Foundation; reviews proposals and develops Memorandum of Understandings, distributes funds, analyzed grantee’s progress, and developed reports for funders that promoted future funding.

Provided case management to formerly homeless individuals through assessing, planning, implementing, coordinating, monitoring, and evaluating the options and services required to meet clients’ mental health and human services needs related to housing and improving housing stability; conducted weekly apartment inspections, inputted case notes and other interactions with consumers into the BUI database.

Provided case management activities necessary to meet the needs of clients assigned to a consolidated care team. Developed individualized care management plans, implemented care plans through brokering and coordinating services, and evaluated all clinical outcomes to ensure that services were provided in a professional, comprehensive, and cost-effective manner.

 Determined residency status for tuition purposes, accurately retrieved high school/GED and college/university codes from BANNER based on application information; accurately coded desired program of study as well as admit and student type from valid codes in Banner; entered data including address, bio/demographic information, high school/GED information, prior college/university information, test scores, Georgia requirements information, immunization, and application information; conducted research to determine school accreditation.
