With over 10 years of experience in medical social work, I am passionate about providing end-of-life care and support to patients and families facing terminal illnesses. I am a Licensed Clinical Social Worker and an Advanced Palliative Hospice Social Worker - Certified (APHSW-C). I currently work as a Hospice Social Worker at Dignity Health, one of the largest health care systems in the nation, where I share their vision of delivering compassionate, high-quality, and affordable health services to all.In my role, I manage a large caseload of high-acuity patients and promote positive coping through identification of patient/family strengths, recognition and intervention of negative social determinants of health, reinforcement of adaption and resilience at end-of-life, and coordination of resources and referrals.
