In Cardiac Rehab, my primary job was to educate cardiac patients, in an inpatient setting, on how to manage their own health. My focus was especially on patients with Congestive Heart Failure, STEMI, NSTEMI, or recent CABG. The goal of this education as to ensure patients could take care of themselves once they were discharged from the hospital. My secondary goal was to decrease cardiac patient's length of stay by daily ambulation. Daily ambulation prevented the patient from deconditioning in the hospital and built the patients strength up for discharge. If a patient required home assistive devices or assistance at home, I would organize this through working with the patient's case manager. While working inpatient, I also worked in Outpatient Cardiac Rehab. The goal of Outpatient Cardiac Rehab, is to keep patients from returning to the hospital. The patients would return for 12 weeks (or 36 visits), where I would continue to teach the patients the importance of self care and develop an exercise program with the patient. I would then lead the patient through the exercises to rebuild the patient's cardiac strength.
