    In its third iteration, J2SI is an innovative and evidence based case management service for people experiencing long-term homelessness in Melbourne, Victoria. It differs from transitional service as it provides the time needed for people to access permanent housing, as well as improvement to their quality of life. Staff have the ability to work intensively with small case-loads, and use a key worker and team approach to delivery individualised support for 3 years. As a Program Coordinator, I am able to; - Providing Leadership and support to the J2SI Intensive Case Managers through supervision, training and secondary consultation.- Working collaboratively with the manager and team leader to contribute to a high functioning team. - Assist program team to embed program targets and support team to achieve outcomes. - Support Quality framework, reporting and continuous development. J2SI is funded through a Social Impact Investment, providing opportunity to test out new funding models to address the issue of chronic homelessness, through support of 5 domains.- Sustained Housing- Social Inclusion- Health and Well-Being- Economic Participation- Independence
