Supervise team of 8-10 Coordinated Services Specialist II and Coordinated Services Specialist III.

Supervise group of 2 to 3 Coordinated Services Specialist II. Perform duties of Coordinated Services Specialist II, and provide backup for full team of 8-10 Specialist when team leader for region is unavailable.

Conducts broad-based assessments for individuals and families and identifies client service needs. Collaborates with clients and other human service professionals for prevention, early intervention, and self-sufficiency oriented service options, drawing heavily on family and community resources, as well as public sector services. Develops initial service action plans. Makes appropriate resource referrals and service linkages. Advocates for and assists families and individuals in obtaining services. Provides service coordination, case management, and referrals to ongoing community case management and intensive case management services in selected circumstances. Identifies service gaps and makes recommendations for needed changes. Attends multi-disciplinary meetings with other human services professionals to coordinate wrap-around services and case planning. Utilizes automated systems for case documentation/referrals/coordination and advanced telephone technology in all facets of work.
