CurrentInitiate, develop and maintain relationships with service resistant youth 16-24 years of ageAssess the needs of and establish ‘plans of care’ for clientsAssist clients in identifying attainable short and long-term goals, strategies for meeting these goals, and an action plan for implementationAssist clients in exiting street life, maintaining independence in the community, and/or accessing supported livingProvide youth with practical resources including food, clothing, transportation etcEnsure adherence to and role model Covenant House principles and core valuesShelter Service to youth age 16-24Conduct intake/admission interview, assessment and develop and manage through daily contact, and individualized Client Plan with youth on caseload for the duration of their staySupport youth with daily routines in the highly structured environmentAssess and address the immediate needs of either resident and non-resident youth, especially those who are seeking support when other service providers are closedAddress serious behaviour, mental health, emotional, and addictions issues through counselling, implementing Special Covenant, defusing/managing critical incidents, supportive relationships, and referrals to other CHV programs and/or community service providersAssist youth with accessing community support networksEscorts to community resources when requestedCoordinate youth meetings and activitiesManage crisis intervention and counselling including briefing and debriefing for youthAssist in admission/intake interviews and assessments as neededAccompaniment to appointments, visitation in hospital or independent living situation, 1 to 1 support, risk assessments, supportive counsellingGreet clients upon entry to the Drop in, introduce the services provided, orient new clients, complete program intake/admission and assessment, and establish plans
