The EngageWell IPA Director of Programs is a strategic leadership position with the unique opportunity to define and implement innovations that drive true value for health care plans, leverage the knowledgebase of community-based providers, and improve health care for tens of thousands of historically disenfranchised New York Medicaid beneficiaries. Professional Highlights:- Managed NYC’s largest Ryan White HIV Care Coordination Program, a $1.5M population health program supporting improved health outcomes, engagement in care, and quality of life among high-cost patients with uncontrolled HIV - Recruited, trained, and supervised a diverse project team of 20, full-time employees: Community Health Workers, Data Manager, and a Treatment Adherence Clinician- Cultivated strong relationships with Clinic Managers, Medical Directors, and outpatient medical and behavioral health providers to facilitate clinical integration, patient referrals, data collection, and quality improvement initiatives- Completed regular case audits to assess fidelity to program design, ensure achievement of key performance metrics, and inform staff professional development needs

Health Leads is a national organization with the mission to catalyze the healthcare system, one that addresses the social determinants of health and racial health disparities as a standard part of quality care. Professional Highlights:- Launched a $1.1M social determinant of health program operating in a high-volume, public hospital (HHC) in Brooklyn- Supervised and coached undergraduate Health Advocates from New York University, doubling cohort size from 15 to 30 students each semester- Doubled the number of low-income families served, from 250 to 500+ each year, while tripling the number of successful referrals to public benefit and social service programs, from 300 to 1,000+ annually- Collaborated with community-based leaders and clinic leadership to build streamlined screening and referral systems- Analyzed and presented monthly ROI performance data to Medical Director, medical teams, and hospital C-suite admin
