Investigation input started at the crash site and resulted in analysis and recommendations for the Hazard Entry, Recovery, Escape and Survival phases of accidents. I developed, applied and published my own accident investigation and prevention methodology, which is still used as a key part of the UK military's flight safety approach. This role also included multiple front line operational investigations. -
